Skilled Care at Home
- Skilled Nursing: vitals monitoring, wound care, chronic condition support, medication education.
- Physical Therapy (PT): mobility, strength, balance, fall-prevention programs.
- Occupational Therapy (OT): safer bathing, dressing, kitchen tasks, energy conservation.
- Speech Therapy (SLP): swallowing strategies, communication exercises as prescribed.
- Medical Social Work: community resources, care coordination, advance planning support.
Daily Living & Family Support
- Home Health Aides: bathing, grooming, toileting, dressing, light housekeeping.
- Companionship: meal prep, errands, social engagement, routine building.
- Caregiver Coaching: safe transfer techniques, medication reminders, equipment use.
- Respite Visits: short-term relief so family caregivers can rest and recharge.
How Independence Care Works
- Assessment: a nurse or therapist visits your home to understand needs, goals, and safety.
- Care Plan: we coordinate with your physician to create a schedule and service mix.
- Care Team: matched professionals deliver visits and adjust as you progress.
- Progress Updates: outcomes tracked; families and providers receive timely reports.
Emergency care is not provided. Call local emergency services for urgent or life-threatening situations.
Safer Home, Stronger Independence
Rug and cord checks, grab bars, lighting, and balance exercises.
Reminder systems, organizer setup, and education per provider orders.
Shower chairs, reachers, and kitchen aids recommended by OT.
Eligibility & Referrals
Services may require a physician’s order and clinical eligibility. We coordinate with hospitals, primary care, and specialty providers to support safe transitions home.
